Form 2

This form has been filed for declaration and nomination under the major schemes of Employees Provident Fund and Employee Family Pension. It should be filed by an employee when he is involved in an entity. The form should be submitted with Form 5. Form 2 is divided into two separate parts:

Part A
Part A of Form 2, specifically those who receive EPF balance, are nominated in the event of their death. The following details of nominee should be included in this section of the form:
